When the veteran was ready for discharge, the replacement resident dictated a second discharge report which omitted any reference to the CT scan, one way or the other. Due to problems in the radiology department the CT scan was performed, but not read for more than a week after the veteran had been discharged to his home which was several hundred miles away. The veteran returned to the West Haven VA for his postop visit more than a week after he had been discharged. The resident, who saw the veteran at the post op follow up visit, did not follow up on the issue of the CT scan.
